FMCSA Carrier Onboarding Checklist for Brokers & 3PLs (2026)
Carrier onboarding is the broker's first line of defense against fraud, claims exposure, and routing-guide chargebacks. The FMCSA gives you the data; how you use it determines whether you find out about a problem carrier before or after the load goes missing. This checklist is what we run inside Hayes on every new carrier signup.
1. Verify operating authority
Pull FMCSA L&I (Licensing & Insurance) on the MC/MX number. Confirm: authority is ACTIVE for property (not just household goods), not revoked, and at least 12 months old (most claims fraud rings spin up <90-day authorities).
Cross-check the DOT number against the MC. Mismatches between the carrier's stated DOT and the L&I record are the #1 double-brokering red flag.
Save a PDF of the L&I lookup with timestamp — this is your evidence trail if a claim turns into litigation.
2. Insurance verification
Minimums per FMCSA: $750K liability for general freight, $1M for non-bulk hazmat, $5M for bulk hazmat. Most brokers require $1M auto + $100K cargo as a contract floor.
Get the certificate directly from the carrier's agent, not from the carrier. Email forwarded COIs are the easiest fraud vector.
Name your brokerage as a certificate holder so you're notified on cancellation. Set up auto-alerts 30 days before expiration.
3. BOC-3 process agent on file
Every authorized motor carrier must designate a process agent in every state — verify the BOC-3 is current. No BOC-3 = authority gets revoked = your load is suddenly hauled by an unauthorized carrier.
4. CSA Safety Measurement scores
Pull SAFER and SMS. Disqualifying signals: Unsafe Driving BASIC above the intervention threshold (65th percentile general / 60th hazmat), or a recent unsatisfactory safety rating.
Set a hard cap (most brokers: 75th percentile in any BASIC) and a soft cap (60th) for senior approval. Document the rule, apply consistently.
5. Fraud and double-brokering screening
Cross-check carrier email/phone against Carrier411 and FreightValidate watchlists.
Reject @gmail / @yahoo domains unless the MC was issued >24 months ago. New authorities using free email are the #1 fraud pattern in 2024–2026.
Verify the carrier's stated phone number matches the FMCSA contact on record — fraud rings register new phones the day they spin up the authority.
Set an out-of-pattern alert: a carrier hauling a lane they've never run that's >500mi from their HQ deserves a phone call to the dispatcher before tender.
6. Carrier packet contents
Signed broker-carrier agreement (your terms — payment timing, claims process, accessorials, indemnification).
W-9 matching the legal name on the MC authority.
Factor NOA (Notice of Assignment) if applicable — pay-to address must match.
Voided check or ACH form for direct deposit.
Insurance COI naming your broker as certificate holder.
Driver list with CDLs (for high-value or hazmat lanes).
Onboard a new carrier in under 15 minutes
- 1
Capture MC + DOT
Have the carrier submit MC/DOT + W-9 via a tokenized intake form. Hayes auto-pulls FMCSA L&I, SAFER, and CSA in <30 seconds.
- 2
Verify insurance
Hayes pings the agent listed on the COI for confirmation. If it doesn't match the COI on file, the carrier is paused for manual review.
- 3
Run fraud screening
Auto-check Carrier411, FreightValidate, email domain age, and prior-load patterns. Flag anything that breaks your rules.
- 4
Send the packet
E-sign broker-carrier agreement + W-9 + ACH in one flow. Counter-sign on your side and lock the version with a SHA-256 hash.
- 5
Activate with limits
First 3 loads are auto-rate-capped and require dispatcher approval. Lifted after a clean run.
Frequently asked questions
How long does manual onboarding take?
Industry median is 1.5–3 days for brokers running paper/email packets. Automated onboarding (Hayes, RMIS, MyCarrierPortal) is 10–20 minutes if the carrier has docs ready.
What's the biggest fraud red flag?
Authority <90 days old + Gmail/Yahoo contact email + a carrier name that closely mimics an established carrier. That combination accounts for the majority of double-brokering reported to TIA.
Do I need RMIS or MyCarrierPortal if I use Hayes?
No — Hayes performs the same FMCSA L&I, insurance verification, CSA monitoring, and fraud screening, integrated with your TMS so you don't pay a per-onboard fee.
What about international carriers (Mexico, Canada)?
Hayes pulls the equivalent records (SCT for Mexico, NSC for Canada). Cross-border carriers also need C-TPAT/FAST verification for expedited customs, which we attach to the carrier record.
